Do all mice (both Kempston and AY) have two buttons or are there difficult cases? Is it worth
to attach separate functions to them?

In KempstonMouse 2 at least. The right one is usually Cancel :)
(port #FADF bit 1), and the left one is Fire (bit0). The third button
is almost always superfluous.

I make KM interfaces with 3 buttons. I mean, 3 buttons are possible.

Well, why? IMHO you can also interrogate the middle button via bit2.
And you can come up with any purpose for it, it depends on the specific case.

AY has only two, Kempston they say there are 3.

On the left and right - of course it’s worth it. The middle one on KM can be adapted to
something like Shift+Left.

Theoretically, Kempston could have three buttons, but you shouldn't count on it. Kempstons with one button have never been found in nature :)
